We are an adult rehabilitation service for people with long-term health conditions and / or disability support needs. We aim to minimise the impact of disabilities and/or personal health issues, and work in partnership with clients and their family/whānau to enable optimal independence and promote self-management. We work closely with internal and external providers to deliver patient-centred continuity of care.

Rehabilitation is offered in the home for clients who are unable to leave the home due to physical limitations. Rehabilitation can be delivered in community and clinic settings to meet clients’ needs. Telehealth services are provided where appropriate. 

What to expect
Physiotherapists help clients improve their physical activity level and quality of life. We promote developing self-management skills and functional activities. By providing you with knowledge, advice and support we can help you to understand and manage your condition. 

The physiotherapy assessment involves asking questions followed by specific physical testing. We work closely with other health providers including the GP, and agencies outside of ADHB. 

Adult Community Physiotherapy is unable to provide services for ACC clients, clients receiving physiotherapy from another service for the same condition, clients who require an immediate response to mitigate risk, and clients residing in residential care facilities or on Waiheke Island.

Services provided Physiotherapy Physiotherapists diagnose and support people to regain mobility and reduce pain caused by an injury, disability or health condition such as arthritis.
